Consent
- The purpose of the film and how my contribution may be used were explained to me before filming, and I had the chance to ask questions.
- I agree to be filmed and recorded. The production may edit my contribution and use it in the film, its trailers and promotion, in all media, worldwide, without time limit, respecting the identification choices above.
- I may withdraw my consent by writing to ___ before ___ (date). After that date, or once the film is finished, the production may keep using what was filmed.
- My personal data is used only for this project, kept securely for ___ and not sold to anyone.
- What I say is, to my knowledge, true. Any photos, videos or documents I give the production are mine or I have permission to share them.
- I am not owed any payment beyond the fee stated above.
- Minors and vulnerable people: a parent, guardian or legal representative must also sign.
